For prepaid account:
1. MIN-kitted/bundled based on CSP/Optima 360 record
2. With OR (proof of purchase) and beyond 24 months from date of purchase
3. If subscriber cannot provide OR as proof of purchase, he/she can present the following:
Present Valid ID and Notarized Affidavit of Loss with the following information:
• Name of Subscriber
• Date of Purchase
• Device Model/Type
• IMEI of device
• Reprinted copy of Official Receipt containing pertinent details
• For select prepaid handsets availed through Smart’s handset partners:
1. Beyond 24 months from date of purchase (as indicated in the OR).
