Help PA HELP PO !!

Hello! I understand your concerns about your lost documents and the fear of getting an Affidavit of Loss. Here are some steps you can take to address your situation:

1. Contact Your Previous School: Try reaching out to the school where you previously enrolled for college. They may have a copy of your submitted documents on file. Explain your situation and ask if they can provide you with a duplicate set of your SHS card/Form 138 and a copy of your Good Moral Certificate.

2. Request for Duplicate Copies: If your previous school is unable to provide you with duplicate copies of your documents, you can ask them for guidance on how to proceed. They may have specific procedures for requesting duplicates or issuing replacements.

3. File an Affidavit of Loss: If all else fails, and you are unable to retrieve your documents from your previous school, you may need to proceed with obtaining an Affidavit of Loss. This legal document will state that your documents were lost and that you are not in possession of them. This is a common procedure for situations like yours.

4. Explain Your Situation: When you apply to a new university, be honest about your situation regarding the lost documents. Explain that you have taken the necessary steps to address the issue, such as requesting duplicates or filing an Affidavit of Loss. Most institutions understand that situations like this can happen, especially during the pandemic.

5. Follow the New University's Procedures: Make sure to check the specific requirements of the university you plan to enroll in. They may have different procedures or additional steps for cases like yours. It's essential to be proactive and communicate with the admissions office to ensure a smooth enrollment process.

Remember, honesty is key in situations like this. By being transparent about your circumstances and taking the necessary steps to address the issue, you can demonstrate your commitment to pursuing your education. Good luck with your enrollment process, and I hope everything works out for you!
