What information are required to register SIMs?
The end user needs to provide the following information:
  • Full name
  • Date of Birth
  • S e x
  • Address
  • Government ID with photo
  • Declaration that the ID presented are true and correct

What kinds of identification cards will be accepted to register SIMs?
The following are the official ID cards that will be accepted in registering SIMs:

  • Passport
  • National ID
  • Social Security Service ID
  • Government Service Insurance System e-Card
  • Driver’s license
  • National Bureau of Investigation clearance
  • Police clearance
  • Firearms’ License to Own and Possess ID
  • Professional Regulation Commission ID
  • Integrated Bar of the Philippines ID
  • Overseas Workers Welfare Administration ID
  • Bureau of Internal Revenue ID
  • Voter’s ID
  • Senior citizen’s card
  • Unified Multi-purpose Identification Card
  • Person with Disabilities card
  • Other government-issued ID with photo

What information are needed for me to register my SIM?
Subscribers are required to provide and submit the following information for them to register
their SIM:
1. Mobile Number with Serial Number (Serial number to be provided by SMART)
2. Full Name
3. Date of Birth
4. Complete Address
5. S e x
6. Upload a valid government-issued ID (please click here to see the list of valid IDs)
7. Selfie Photo

What IDs are acceptable for me to be able to register my SIM?
Under the Law, acceptable Valid IDs or any similar form of documents with photo that will verify the identity of the end-users include, but are not limited to, the following:
1. Passport;
2. Philippine Identification;
3. Social Security System ID;
4. Government Service Insurance System E-Card;
5. Driver's License;
6. National Bureau of Investigation Clearance;
7. Police Clearance;
8. Firearms' License to Own and Posses ID;
9. Professional Regulation Commission ID;
10. Integrated Bar of the Philippines ID;
11. Overseas Workers Welfare Administration ID;
12. Bureau of Internal Revenue ID;
13. Voter's ID;
14. Citizen's Card;
15. Unified Multi-purpose Identification Card;
16. Person with Disabilities Card;
17. Other valid government-issued ID with Photo

Sa DITO

Subscribers will be required to provide their personal information:
  • Full Name
  • Date of Birth
  • Gender
  • Present/Official Address
  • See FAQs for the list of accepted IDs
For minors:
The SIM card should be registered under the parent/legal guardian's name along with:
  • Any government-issued ID of the parent or guardian
  • Consent of minor's parent or guardian to register the SIM card
For businesses, requirements are as follows:
  • SEC / DTI Certificate of Registration
  • Board Resolution designating the authorized representative or a Special Power of Attorney (SPA)
For tourists:
  • Passport
  • Proof of Address in the Philippines
  • Return ticket to own country showing date/time of departure from the Philippines
For foreigners with non-tourist visa:
  • Passport
  • Proof of Address in the Philippines
  • Other pertinent documents such as
    1. Alien Employment Permit issued by the Department of Labor and Employment (DOLE);
    2. Alien Certificate of Registration Identification Card (ACRI Card) issued by the Bureau of Immigration (BI) or other types of official ID issued by any other visa-issuing agency;
    3. School registration and ID for students; or
    4. For Persons of Concern or POCs, the type of travel or admission document validly issued by the Department of Justice.
