• Philhealth ID (Digitized/PVC or Laminated Cardboard with Barcode)
• Voter's ID
• TIN ID (Laminated)
• Government Service Insurance System (GSIS) e-Card
• Overseas Workers Welfare Administration (OWWA) ID/ OFW e-Card

For Filipino minors aged 7 to 17 years old

1. Primary ID (any of the following)
   • School ID (issued within the current school year and signed by the school principal or head of the educational institution)
   • Passport
   • Driver's License
   • Postal ID
   • Philippine National Identity Card
2. Parent/ Guardian's Valid ID (based on accepted primary IDs)
3. Signed Parental Consent form
4. PSA/NSO Birth Certificate
5. Proof of legal guardianship (if applicable)

Kapag po nakapagregister na kayo may 20pesos ng laman agad yun account nyo then kapag naiupgrade nyo na account nyo saka makukuha yun additional 80 pesos.
