It should be: “The patient died despite [the fact that he had received][having received][receiving][having] the best medical care.”

The rule is that we shouldn’t say “despite he had received ….”

The word following “despite” should be a noun (e.g., “the best medical care”) or a gerund, which acts as a noun.
